Overseas Guest Appointment
一般看完門診都要等上一段或長或短的時間才排得到手術，不過我們可以給住在國外的網友方便，讓門診和手術的間隔縮短到兩天以內，但必須事先安排，故請您將以下表格填妥後送出，醫師在收到後就會儘速為您評估狀況並給您建議。
After a regular clinic visit, patients usually need to wait for a period of time, long or short, before scheduling surgery. However, we can provide convenience for patients living abroad by reducing the waiting time between the clinic visit and surgery to within two days. This requires prior arrangement, so please complete and submit the form below. Once received, the doctor will evaluate your condition as soon as possible and provide recommendations.

僅提供給住在國外的網友預約手術之用，國內的網友請先來電(02)2755-0077預約門診，門診後再預約手術即可。
This form is exclusively for overseas guests to schedule surgeries. For domestic patients, please call (02) 2755-0077 to book a clinic appointment first, and then schedule the surgery after the consultation.
STEP 2
預約日期 Appointment Date
隆乳、抽脂等全身麻醉手術提前二個月，如雙眼皮、眼袋等局部麻醉手術提前四個月
手術必先經過門診諮詢評估，因此門診日期不可超越手術日期
For general anesthesia surgeries such as breast augmentation or liposuction, appointments must be made at least two months in advance. For local anesthesia surgeries such as double eyelid or eye bag procedures, appointments must be made at least four months in advance.
A consultation and evaluation are required before surgery; therefore, the clinic appointment date must precede the surgery date.

For intimate area surgeries, photo uploads are not required.
